Ground Glass Appearance of lungs

Ground glass appearance is not seen in:

A. Hyaline membrane disease
B. Pneumonia
C. Left to right shunt
D. Obstructive TAPVC

Answer is C. Left to right shunt.

Left to right shunt shows hyperemic or plethoric lung fields and ground glass appearance is not a feature here.

Characteristic radiograph of obstructive TAPVC consists of a normal sized heart with severe pulmonary hypertension, resulting in the ground glass appearance of the lung.

The differential diagnosis to this includes:
  • Persistent pulmonary hypertension
  • Respiratory distress syndrome
  • Pneumonia (bacterial as well as meconium aspiration induced)
  • Pulmonary lymphangiactasia
